I was probably super lucky because I got my first Mahatheb two days ago after killing Mummy two times :p
Yooo, I made a long research on Mahatheb. I finished it for some time though, cause I switched to playing on one of new servers. But I'll provide you with numbers in general, so all of you know what to expect:
I hunted it on 4 servers for 1.5 month, relogging to from one server to another after killing vampire/bonebeast.
Overall I killed 9011 vampires, 961 vampires and 7 Mahathebs.
So I killed Mahatheb every 1287 vampires and 138 boenebasts. But of course it's just a stat. I know that you can even kill it twice in a row if you are extremely lucky.
What's funny: during that 1.5 month I killed it 4 times on Bona, 2 times on Nadora, 1 time on Monza and 0! on Damora. Even though on Damora I killed 2603 vamps/bbs which is more than I did on Bona and Monza! On Bona I killed the fewest mobs - just 2200, so you see that it is just pure luck.
And I think we can assume that there are no additional factors that determine Mahatheb respawn. It's just random, like rolling the dice, you will eventually get 3x6 in a row - same Mahatheb, you'll eventually got him.
So overall I killed 9866 monsters, of which 7 were Mahathebs, but I still think that Mahatheb has 0.1% chance to resp so on average he should resp every 1000 monsters.
I haven't looted anything interesting unfortunately, but I wish you all good luck in discovering any new mysteries of Mahatheb!

I bought a RP only to farm Mahatheb in Astera. I named it Mahatheby :))
A couple of days ago We were able to get him 3 times in the same day. In total I think we got him 9x
Interestingly this happened the day before Horestis canopic jars world change.
Do we have any lore that connects Horestis to Mahatheb?
